Doing your Kegels "for months" and nothing changes? Here is why

Published : 2026-07-18

It is THE sentence every woman with leaks has heard: "just do your Kegels." You took it seriously. In the car, at your desk, in the grocery line. Months of diligent squeezing... and the leak-when-you-laugh still shows up on schedule.

The tempting conclusion is "my case is hopeless." The far more common reality: Kegels were the wrong tool, or done the wrong way. Here are the four scenarios that come up again and again.

1. Technique: squeezing, sure, but squeezing what?

A large share of people "doing their Kegels" are actually contracting glutes, thighs or abs, or even pushing down instead of lifting up. The right contraction is subtle: a gentle inward lift, as if holding back gas, while the breath stays easy. If nobody has ever checked your technique, this is suspect number one.

2. A pelvic floor that is too tight, not too weak

This is the counter-intuitive one: a hypertonic pelvic floor, clenched around the clock, that can no longer contract effectively or relax. In that situation, adding Kegels is squeezing a fist that is already closed; it can even make symptoms worse. The game plan then runs through release work and pelvic floor physiotherapy, not more squeezing.

3. The problem is not (only) strength

Sudden urges that run your day point more to an overactive bladder than to weak muscles; leaks around menopause can have a hormonal component; some medications and habits play a role too. Kegels stay useful in many of these situations, but on their own they will not fix a cause that is not muscular.

4. The consistency (and intensity) no schedule allows

A serious strengthening protocol means maximal contractions, repeated, several times a week, for months. In real life, between work and family, most people plateau well below that. This is exactly where assisted strengthening fits: the Emsella chair at the partner pharmacy triggers the equivalent of about 11,200 contractions in 28 minutes, at an intensity you cannot produce voluntarily. It is not magic and it is not for everyone: it is a way to get the training volume your schedule cannot, once low tone is confirmed.

Your questions

Your questions

How long before concluding Kegels "are not working"?

With good technique and consistency, first changes are expected within 6 to 12 weeks. If you have been diligent for 3 months or more with zero improvement, it is time to have the technique and the cause assessed rather than push on blindly.

How do I know if my pelvic floor is too tight rather than too weak?

Clues include pelvic pain or pain during intimacy, difficulty starting urination, constipation, and a feeling of constant tension. The actual diagnosis belongs to a pelvic floor physiotherapist, who assesses tone at rest and under effort.

Does Emsella replace Kegels?

No. The chair produces a volume of contractions you cannot reach on your own, but Kegels keep their role between sessions and afterwards, to maintain the gains. One is an accelerator, the other a habit.
